I am the owner of this pet and have all legal and financial responsibility. I hereby give PLF the right to make all decisions for your pet during the time of leaving (listed above). I understand that any veterinary bills accumulated will be my responsibility and must be paid at the time of pick up or my pet will not be returned to me. I understand that PLF is under no obligation to notify the owner before my pet is taken to the vet clinic and that I may just get a phone call afterward notifying me of the cost and pet’s condition in order for the best care to be given to the pet.

If my pet requires flee treatment, any first aid care (open soars, etc), nutritional supplements, or any other "special needs" while in PLF care, the cost of the product will be charged to the owner at the time of pick up--receipt will be provided as proof of cost.

If my pet is injured while in the care of PLF, I will not hold the owners of PLF responsible.

If my dog causes harm to another pet while in the care of PLF, I will accept full monetary responsibility for the damage to the other pet. If I should choose to return sooner than the agreed time of reservation, customer will receive no refund. All money is paid up front. I agree to call one hour ahead before arrival at PLF for all drop off and pick ups.

By signing below, you the owner, relieve PLF of any liability in relation to the side effects of medicine or any "lack of coverage" they offer even though we use the top of the line vaccine out there. Proper and legally required documentation will be provided of any medications given. The owner must understand that it is illegal for PLF to administer the Rabies vaccine and that this must be done by a licensed veterinarian; but while your animal is here, we are happy to make an appointment and take the dog for you. Also, PLF does not have a licensed groomer on the premises. So, depending on the grooming needed, the price will vary based on the need of the groomer. If you wish for us to call you with a quote of what it will cost before having it done, please check here:______. All costs for the last two listed--grooming and rabies must be paid in cash in advance. For all other services, owner's MUST give a one hour notice/pet before planning to pick the pet up to allow time for the listed items to be done.

Puppy Luv Farms Rules for Leaving Your Pet

By signing below I, as the above listed owner, have read and am agreeing to the below rules. I am also acknowledging that I have also been offered/received a copy for my records. Thank you for allowing us to serve you and your pet. Please refer us to your friends:

For all bounced checks there is an additional $40 fee.

All clients must give a 1 hour notice before drop/pick up. Remember, this is our home. Please be courteous.

All fees are to be paid up front for the agreed reservation time. If I choose to pick my pet up early, no refund will be given. See our website for holiday fees to be applied to each account (we charge per family rather than per dog.)

If I make a reservation and do not give proper 48 hour notice, the fees for that reservation will be on my account to be paid the next time I choose to leave my pet at PLF.

If my pet is not properly vaccinated, flea protected, or heart wormed, this will be done during my pet’s stay and the cost will be added to my bill which I am responsible to pay.

I understand that the owners of PLF are not the only caregivers here at PLF, and that all caregivers have met required approvals by PLF.

I understand that my pet may be taken off the grounds for a walk on a safety harness we provide or in a vehicle leaving PLF property only with the owner.

My pet will be kenneled at night.

If my pet is injured, all vet bills will be compensated to PLF, or I will not get my pet back. I leave my pet here at my own risk.

If my pet harms another animal, I will assume the cost of all vet bills, or not get my pet back.

Your pet’s specific food brand will no longer be provided. There is now a flat rate of $10/wk applied to each account for pets under 30 lbs. Pets 30-50lbs. will receive a charge of $15/wk. Standard food fed here is Iams in the green bag.

Drop off and pick up may be arranged before/after business hours along as arrangements are made ahead of time. But you will be charged for another 24hr fee if you are past the time allotted from drop off (ie. Dropped Mon. at 2pm, picked up Fri at 4pm. You will be charged for Fri. unless you are there by 2pm) For any before/after business hr. services, there is a $5 fee per service (example: phone call $5, drop/off $5, Pickup $5 etc) Leave a message for all after-hour needs. We will call you back.

There is a sign in/sign out sheet on the coffee table for each customer to sign. If your information on your application changes, it is your responsibility to notify us. This will save you a lot of time when coming and going. Please write the date/time of drop off or we will automatically start the time at 8am. WE NEED YOUR EMAIL LIST, in order to cut back on postage so please email or call me ASAP with that!

Beginning Jan 1, 2010, there will be a flat rate of $5/wk applied to each account for snacks given twice daily to your pets while they stay here. This does apply to day care as well. If you wish to avoid this charge, please provide treats for your pets.

You agree to abide by rules presented and understand that PLF and all associates have complete ownership of your pet while in our care and that you will not hold PLF or it associates legally responsible for any occasion.
